A case of primary myopericarditis in a previously healthy 19-year-old man is described. The patient presented with chest pain, fever, dyspnoea and arthalgia. Cardiac tamponade developed on the fifth day with recurrence on the seventh day, while meningitis never developed. Blood cultures grew Neisseria meningitidis serogroup W135. The patient was treated with penicillin, pericardiocentesis and prednisone. He recovered without sequelae.
