PIP: After successful experiments on rabbits during 1975-1976, tubal sterilization with silver clips was clinically tried on 1,182 women and results followed for up to 3 years. The women in this series comprised of 1135 villagers and farmers (96.02%), and 47 government or nongovernment workers (3.98%). Their ages ranged between 20-40 years with 14.97% between 20-25, 63.03% between 26-30, 18.02% between 31-35, and 3.98% between 36-40. The silver clips were handcrafted each containing 99.98% white silver. The surgical procedure, indications and contraindications are similar to those for other tubal ligation technics. A silver-clip clamp (modified from the cerebral silver-clip clamp) is used for applying the clips. During the operation, with isthmus (about 2.5-3.0 cm from the uterine horn) and places the clip completely over the whole transverse diameter of the uterine tube. Pressure is then applied through the clamp and, after releasing clamp pressure, the silver clip is firmly in place. 2 clips (.5 cm apart) are used per uterine tube. To prevent failure, clips should not be applied at the uterine horn or at the ampulla or infundibulum of the uterine tube. No serious complications were observed in this series; patients were generally discharged from the hospital 1-2 days after surgery. During postoperative follow-up, 10 women became pregnant, constituting a failure rate of 0.85%. The major reason for failure was insufficient tightness of the clips. In order to reduce slippage or disengagement of the clips, an improved machine-made silver clip of increased width (2.5 mm) was used. It had small anti-slippage protrusions and only 1/uterine tube was required. Preliminary results on 26 women showed complete blockage of the uterine tube in 12, 8 weeks after surgery.
