This article describes the results pertaining to fertility and family planning from a 1985 survey conducted in the catchment area of Chogoria Hospital in central Kenya. Current contraceptive prevalence was found to be quite high, 43 percent as opposed to 17 percent for Kenya as a whole. The total fertility rate of 5.2 births per woman was 2.5 births lower than the national rate. Very few women reported wanting to have large numbers of children or thinking that fate or God should determine family size. Although these data cannot conclusively demonstrate that the family planning program operating in the area has been responsible for reduced fertility there, they do provide some support for this hypothesis.

PIP: A population-based survey conducted in the catchment area of the Chogoria Hospital Community Health Department in central Kenya in 1985 revealed high levels of contraceptive use and marked declines in fertility unprecedented in rural Kenya. Contraceptive prevalence in the catchment area was 43% compared to 17% for Kenya as a whole, while the total fertility rate of 5.2 births/woman was 2.5 births lower than the national rate. 89% of respondents in the Chogoria survey spontaneously reported knowledge of oral contraceptives and 75% reported awareness of the IUD, attesting to widespread knowledge about modern family planning methods. Moreover, these 2 methods accounted for 62% of all contraceptive use in the catchment area Half of the married, fecund respondents in Chogoria wanted to have no more children--a statistic about 50% higher than that found in the rural sample in the 1984 Kenyan Contraceptive Prevalence Survey. A proximate determinants model indicates that fertility in the catchment area is only 62% as high as it would be in the absence of contraception, compared with 87% for Kenya overall. The fact that mean parity by age does not differ dramatically between the catchment area and the older age groups (40-44 years) suggests that the lower fertility in the former area is a recent phenomenon, most likely a result of Chogoria Hospital's family planning outreach activities. Considered essential to the success of these activities are the complete integration of family planning and other services, community involvement in service delivery, a large network of volunteer community health workers, high levels of service accessibility, and the acceptance the hospital has gained over time with the population it serves.
