Aorto-pulmonary window, interrupted aortic arch and abnormal origin of the right pulmonary artery from the aorta, combined with patent ductus arteriosus and with intact ventricular septum, is a very rare and complex cardiac malformation. We report one-stage repair in a 4 day-old neonate, using aortic homograft tissue to reconstruct the ascending aorta.
