Esophagectomy, with gastric pull up replacement, is not uncommonly complicated by leakage from the esophagogastrostomy anastomosis. Occult ischemia of the mobilized gastric fundus is a major etiological factor for anastomotic leakage. Gastric tissue perfusion can be improved by ischemic conditioning ("delay" phenomenon).
