In peripheral vascular disease, iliac occlusions are one of the most difficult lesions to treat, because of the frequency with which dissection occurs from an ipsilateral approach. We describe a technique of subintimal recanalization of iliac occlusions by a combined ipsilateral and contralateral approach.
