The clinical case of an 81-year-old woman with an isolated pisiform dislocation is reported. This is an extremely rare lesion due to the sturdiness of the ligamentous complex which stabilizes the pisiform to the carpus; when it does occur closed reduction is impossible because of the action of the FCU tendon. In symptomatic cases the best therapeutic procedure consists in the surgical removal of the bone, resulting in complete recovery without consequence.
