A marked increase in acceptance of the ovulation method of contraception, together with promising method and user failure rates, were revealed in this study of women to whom a system of combining the ovulation method with the optional use of a diaphragm, advisedly as a menstrual seal, was offered.

PIP: In a 2-year prospective study 280 women were offered a contraceptive method that combined the use of the ovulation method with the optional use of a diaphragm, advisedly as a menstrual seal. In standard brief form with the aid of formal diagrams, the Billings Ovulation Method was explained in depth. The efficacy of the diaphragm as a menstrual seal after douching, and its probable anticancer effect was pointed out along with the fact that the period of abstinence could thus be minimized to the fertile phase -- usually 7-8 days per cycle. The women were warned that if the diaphragm was used during the fertile phase, accurate cervical mucus interpretation basic to the ovulation method would be severely compromised. Method failure rates were quoted as 0.5/100 women years for the ovulation method and 2.5 for the diaphragm. There were 124 responses to a subsequent confidential questionnaire of 48 questions. Of the 124 women responding to the questionnaire, 30% were using the ovulation method as their contraceptive; 20 of this group were also using the diaphragm. The question about the practice of coitus during the menses before briefing was answered as follows: preferred, 1%; as regularly as usual, 17%; occasionally, but not preferred, 45%; and never, 36%. 9 women considered the idea of using a diaphragm during the menses to be very useful; 47 found it useful; 29 not useful; and 21 women thought it a ridiculous idea. 24 women tried the diaphragm as a menstrual seal, and 13 of these found it effective. Significantly more women who had married when they were older than age 19 chose the ovulation method.
