A 66-year-old woman had autopsy-proven miliary tuberculosis, with regional involvement of the meninges of the left Sylvian fissure. Radionuclide studies of the brain demonstrated the unusual findings of increased perfusion to the left hemisphere and linear increased activity corresponding to the left Sylvian fissure. Computed tomography of the brain was initially within normal limits at a time when the radionuclide study was clearly abnormal.
