The anatomy of the spinal subarachnoid space is reviewed with special reference to the work of Key and Retzius, the authors' own cadaver dissections, and experience from operative cases. It appears that despite some variations in the spinal arachnoid anatomy, there are many consistent features that can serve to guide the operating neurosurgeon.
