The authors report a case of fatal pancytopenia associated with sulfamethoxazole-trimethoprim therapy in a patient with folic acid deficiency. The mechanism is a matter of speculation, but the most attractive view is that the inhibiting effect of trimethoprim on dehydrofolate reductase can involve pancytopenia in patients with folic acid deficiency.
