The CT findings in three cases of surgically proven hydrosalpinx following tubal ligation are described. In every case a cystic mass of characteristic tubular shape was detected in the pelvic adnexal region. These findings can be considered diagnostic of iatrogenic hydrosalpinx.

PIP: Although hydrosalpinx is usually a sequela of gonorrheal salpingitis, it may also develop as a complication of tubal ligation by the Madlener method. This paper describes computed tomography findings in 3 patients with surgically proven iatrogenic hydrosalpinx. The symptoms of hydrosalpinx in these women were nonspecific, although vague lower abdominal pains were characteristic. With torsion of a hydrosalpinx, the patients experienced the abrupt onset of severe pain. Physical examination revealed cystic masses in the uterine adnexal region. Computed tomography clearly showed cystic masses of a tubular shape in the uterine adnexal region. These masses differed from multiloculated ovarian cysts by their tubular shape and from intestinal loops by their failure to opacify with oral contrast medium. This article represents the 1st description of hydrosalpinx in the computed tomography literature.
