Surgical therapy of old fractures of the alveolar ridge is indicated if masticatory disturbances exist or are expected. Bony fragments of the alveolar ridge which carry teeth and are consolidated in dislocation are osteotomized, repositioned and fixed by dental splints for more than eight weeks. The guidelines for segmental and en bloc osteotomy in orthodontic surgery are applicable. Defects of the alveolar ridge are treated by vestibuloplasty or reconstructive surgery. It is also possible to combine the two methods. For the reconstruction of the alveolus, recently almost exclusively homologous transplants preserved in Cialit are used. Following reconstruction, additional vestibuloplasty may be carried out three months after the transplantation of bone or cartilage.
