A rare case of an accessory phalanx is presented. It is hypothesized that this anomaly occurred because of an excessive cavitation in the embryonic mesenchyme of the second digit, resulting in an accessory bone and interphalangeal joint. The elongated digit caused jamming within the shoe, resulting in distal pain, a hammered digit, and associated atrophy of the distal aspect of the second toe.
