In today's healthcare delivery system, nurses are accountable for monitoring the quality of patient care. Effective nursing documentation tools can provide the essential element for successfully monitoring specific aspects of care. The extensive use of vascular access devices (VADs) in oncology settings warrants monitoring the standards of care/standards of practice for this important aspect of care. Use of a VAD flowsheet for documenting routine management provides an effective means of documenting, as well as of retrieving, data for monitoring care.
