This paper describes a functional model (gross style) taking available information about the health sector e.g. indicators of health status, types of service utilization, source allocations and placing these quantities into a systematic framework as a tool for planning for health services in a developing area. The major advantages of functional models over trial-and-error for planning have been presented. The functional model described could contribute to the sorting out of desirable from less desirable policy alternatives for a specific population. The output from the model is useful for matching resources to program development, taking into consideration costs and effectiveness.

PIP: A functional model for planning health services in a developing country, taking into account interventions, age-specific disease rates, resources, facilities, effectiveness and costs, that will eliminate the hazards of trial-and-error planning, is presented. The results of the model can be used to rank policy alternatives by effectiveness. The model is particularly useful where vital statistics and death registration are inadequate, and interpretation of health changes is affected by compounding factors. Either aggregated data or published crude death rates and age-specific mortality rates can be used to check the accuracy of regional calculations generated by the model. Variables needed for computation are number of people in each age cohort, attack rate of relevant diseases at each age, case fatality rate from the disease in each age cohort for those who seek care and of those who do not use care, proportion of persons of each age group who seek care, days of disability from the disease in each age group for those seeking care and those who do not. Computers can then calculate health status under each strategy of public health promotion or medical care. Calculation of total mortality is the product of attack rates and age- specific population times the fatality or disability rate. After cost computations, involving utilities, drugs, equipment, supplies, personnel and training are factored in with use rate and client flow, a relationship of cost to utilization of health services is obtained. Output of the model will permit comparison of various program alternatives when effectiveness is compared to budget outlay and alternatives are ranked by health improvement.
