We present an exceedingly rare case of carcinoma ex pleomorphic adenoma (CXPA) arising in a minor salivary gland of the upper lip. We could not identify any reports of CXPA occurring in the upper lip in the literature. This case highlights the need for early surgical review and management of oral lesions of uncertain and/or unpredictable nature.
