Microangiopathic changes in capillary architecture associated with collagen diseases can be visualized by vital capillary microscopy. In our study of 100 patients with collagen disease we found typical capillaroscopic images in 64%. Differentiation in subclasses showed the highest diagnostic value of this method in systemic sclerosis (77%) and rheumatoid arthritis (68%). It was lower in patients with Sharp- and CREST-syndrome (55%) and very low in systemic lupus erythematosus (33%). In summary, vital capillary microscopy is a valid method in non-invasive diagnostic of collagen diseases.
