We report the case of a 20-year-old woman with extrahepatic portal obstruction, anomalous branching of the celiac artery, and intestinal malrotation with duodenal dysgenesis. The patient had no history of neonatal event and had normal liver function. The combination of these abnormalities, rarely reported in the literature, suggests that the extrahepatic portal obstruction in this case might be congenital in origin.
